GKV vs PKV: should a newcomer ever go private? (2026 basics)
Above €77,400 you may switch to private health insurance — and brokers will find you. Why GKV's free family insurance beats most PKV quotes, who PKV actually suits, and why the door back is nearly one-way.
The PKV pitch sounds great — until you do the family math
Private insurance prices your risk today; statutory insurance prices your income forever. Which one wins depends on exactly one question: who else is on your policy, now and in ten years?

Who is even allowed to choose?
As an employee you can leave GKV only if your gross salary exceeds the Versicherungspflichtgrenze (JAEG) — €77,400/year in 2026. Below that, GKV is mandatory and this whole debate is moot. (Self-employed people and Beamte can choose regardless of income — different article, different math.)
Crossing €77,400 is exactly when insurance brokers start calling — their commission on a PKV contract is a multiple of the monthly premium. The pitch you'll hear ("you pay €400 in GKV, we'll insure you for €280!") compares your cost today, alone, young and healthy against a premium designed to rise for the next 40 years.
The family math that decides it
For a single-earner household — the standard setup for many Indian, Pakistani and Bangladeshi newcomers whose spouse arrives on family reunification — GKV is almost unbeatable:
The comparison flips only for people who will stay high-earning, stay healthy-rated, and stay solo on the policy — the classic PKV customer is a young, well-paid single (or double-income couple where both work above the JAEG) or a Beamter with Beihilfe covering half the bill.
The one-way door
The part brokers underplay: you cannot simply come back.
- Return to GKV generally requires becoming versicherungspflichtig again — e.g. your salary dropping below the JAEG. You cannot just "choose" GKV again while earning above it.
- From age 55, returning to GKV is barred in almost all cases (§ 6 Abs. 3a SGB V) — you are locked into PKV premiums through retirement.
- Engineering your way back (reducing hours, salary sacrifice) exists but is a legal tightrope, not a plan.
Rule of thumb worth writing down: choose PKV only if you'd still choose it assuming a spouse, two kids, and a 30-year horizon. If any of those makes you hesitate, stay in GKV and simply pick a cheap, English-friendly Kasse.
Above the JAEG but staying in GKV?
Good news: your GKV premium is capped anyway. Contributions are only charged up to the Beitragsbemessungsgrenze (€69,750/yr = €5,812.50/mo in 2026) — at €90k or €200k gross you pay exactly the same as at €70k.
