Most Blue Card holders earn above the GKV ceiling and can choose either system. 2026 thresholds: €50,700 standard, €45,934.20 shortage. Most first-timers take GKV because switching back from PKV later is hard.

Common pitfalls
Do this
- Activate Krankenkasse against a future IBAN before Girokonto is ready
- Keep employer top-up options in mind for PKV + high-income edge cases
- Switch Kasse after 12 months if you've mispicked — it's a 2-form process
- Use TK's Bonuspass (€150–300/yr cashback) if you're already on TK
Don't do this
- Don't rely on travel insurance for your visa — German embassies reject it
- Don't pick PKV young + childless without modelling old-age premium growth
- Don't forget Zahnzusatz (dental top-up) — GKV covers basics only
- Don't ignore employer payroll transition from student to employee tariff
Changing insurers
You can switch GKV insurers after 12 months of membership with two months' notice. Switching is free and paperwork is minimal. Switching from GKV to PKV requires meeting the income / status thresholds; switching from PKV back to GKV is very restricted after age 55.
