BKK firmus — the 2026 cost leader

  • 2.18% Zusatzbeitrag — lowest of any nationwide Kasse in 2026.
  • Open to everyone (not employer-restricted like some BKK funds).
  • German-language portal only; claims + member service are solid but not English-first.
  • Same statutory benefits package as TK/Barmer — the rate difference is pure savings for a healthy newcomer willing to lose English UX.

TK — the default newcomer choice

Why TK wins for most newcomers

  • Best digital app (German + English)
  • Fastest claims processing — Krankengeld typically paid within 2 weeks
  • Broadest international coverage — EHIC arrives automatically
  • Strong Bonuspass (€150–300/year cashback for health behaviours)

Why you might skip it

  • 2.69% is mid-pack in 2026 — BKK firmus saves ~€25/month on €5k gross
  • You're paying for UX you may not need if you read German fluently
  • No dental full-coverage — dental top-ups (Zahnzusatz) are a separate policy

What nobody tells newcomers

  1. You can switch Kasse after 12 months

    12 mo lock

Switching is a 2-form process — the new Kasse handles everything with your old one. If you started on the wrong one, fix it after a year.

  2. Bonus programmes add up

Most Kassen offer €150–300/year cashback for gym attendance, health check-ups, or vaccination. TK's Bonuspass is the most generous.

  3. eGK card takes 3 weeks

    3 weeks

Emergencies are still covered — the Kasse issues a temporary PDF while you wait for the physical card.

  4. Spouse co-insurance is free

If your spouse earns less than €520/month (Minijob threshold), they're free on your GKV. PKV doesn't match this.

When to consider private (PKV)

PKV makes sense only if all of: income above €77,400/year (Versicherungspflichtgrenze 2026), young + childless, willing to lose GKV family co-insurance, and OK with rising premiums in old age. Switching out of GKV is easy; switching back is nearly impossible after 55.

The switch-over process

  1. Pick your new Kasse online

    5 min

TK, Barmer, BKK firmus websites — all have membership forms.

  2. Submit membership application

    Same day

With your Steuer-ID + current employer info.

  3. Wait for cross-notification

    1–2 weeks

New Kasse notifies your old one automatically.

  4. New eGK card arrives

    3–4 weeks

Physical insurance card mailed to your Anmeldung address.

  5. Employer picks up new Kasse number

Payroll updates automatically next cycle. Done.

For freelancers

If you freelance in Germany, you pay both halves of the GKV premium yourself — on €3,000 net/month, that's ~€570/month on TK, ~€650/month on DAK. Freelancers with high income often switch to PKV — the math flips at ~€4,500 net/month.
