Approbation for doctors & nurses — the honest path from MBBS/BSc Nursing to a German licence (2026)

Medicine and nursing are the heaviest recognition paths Germany has — and the highest-volume ones for India, Pakistan and Bangladesh. The promised 8-week decision hides a document loop, a language exam and a knowledge exam with its own waiting list. Here is the whole route, costed.

The Kenntnisprüfung becomes the DEFAULT route for third-country doctors, dentists and pharmacists

Until now, files burned months proving document equivalence before near-always landing at 'wesentliche Unterschiede' → Kenntnisprüfung anyway. From November the exam comes first and the document fight becomes optional. Veterinarians are NOT included.

Two different mechanisms, one bucket

Both professions end in a state licence from the Land health authority, but the law differs — and case officers care about the words:

Doctor components: application 150–1,000 € per Land · Kenntnisprüfung 400–1,350 € (Schleswig-Holstein lowest, Hessen highest) · Fachsprachprüfung 350–650 € () · translations and document verification 500–1,500 € · optional KP prep courses up to 3,000 €, often fundable via Bildungsgutschein.

Anti-scam, for nurses: the Anpassungslehrgang is usually free to you — employers or the Bundesagentur fund it. And under German law, recruiters may not charge you placement fees (~). Anyone demanding thousands for "guaranteed recognition" is selling you something the state gives away.

The realistic timeline

  1. Documents + verification

    2–6 months

    Degree, transcript, syllabus with hours breakdown, registration (NMC/PMDC/PNC/BMDC…) + good standing, internship/house-job records — sworn-translated. No apostille: Germany rejects apostilles from India, Pakistan and Bangladesh; the authority initiates verification in your home country instead. Missing rotational-internship documentation is the #1 Indian file-killer (~ anecdotal).

  2. Equivalence check by the authority

    4–8 months

    Statutory clock: 4 months for medical professions — but only after completeness. Standard outcome: partial recognition (Defizitbescheid / wesentliche Unterschiede). From 1 Nov 2026, third-country doctors, dentists and pharmacists can go straight to the exam instead.

  3. Language: B2 + Fachsprachprüfung

    2–4 months

    Doctors: B2 general German plus the C1-level Fachsprachprüfung at the Ärztekammer (wait 2–4 months for a slot). Nurses: B2 (some Länder accept B1 at application with B2 by recognition, ~).

  4. Kenntnisprüfung or Anpassungslehrgang

    3–12 months

    KP waiting lists are the known bottleneck — Berlin and NRW especially (~), 3–9 months for doctors. Nurses choosing the Anpassungslehrgang: typically ~4 months theory + 6 months supervised practice (8–11 months; case-dependent up to 3 years).

Doctors

  • Berufserlaubnis (§10 BÄO): a temporary permit, max 2 years, to work supervised as Assistenzarzt while preparing the Kenntnisprüfung — the most-used hack in the community
  • §16d residence permit covers recognition stays; §81a can be continued into §16d entry when only partial equivalence is found

Nurses

  • Work as Pflegehelfer:in during the Anpassungslehrgang at roughly 2,300–2,800 € gross (~) — not your target salary, but real income during recognition
  • Anerkennungspartnerschaft (§16d Abs. 3): enter without prior recognition, work immediately, complete recognition within up to 3 years (A2 German + ≥2-year state-recognized qualification, ~ re-verify parameters)

Country notes

  • India: MBBS from public medical colleges is mostly H+ in anabin; NMC registration + good standing are mandatory; the internship/house-job year counts toward equivalence — document it completely. For nursing, Defizitbescheid is the norm for B.Sc Nursing/GNM; NCLEX or Gulf experience gives no shortcut (); the GIZ/BA Triple-Win programme covers India with a pre-negotiated path ().
  • Pakistan: PMDC registration + good standing mandatory; MBBS → Kenntnisprüfung near-always. BSN → Defizitbescheid the norm.
  • Bangladesh: BMDC registration + good standing mandatory; 3-year diploma nursing faces bigger equivalence gaps (psychiatry/geriatrics hours, ~).
