These are the two modes: cash to you (Pflegegeld) or professional service covered by the Kasse (Pflegesachleistungen). You can combine them proportionally.

The recipient of care must be insured in the Pflegeversicherung — which every statutorily insured resident is (it piggybacks on Krankenkasse). The caregiver can be any close relative living in Germany, including spouses, registered partners, parents, adult children, and in-laws.

Visa path for bringing an elderly parent: if your parent in Pakistan or India needs care you can't provide remotely, the Sonstige Familienangehörige path (§36 AufenthG) allows family reunification for exceptional hardship — dependency on care counts. Separate route, case-by-case at the consulate. Once your parent is resident in Germany and assessed, Pflegegeld kicks in.
How to claim
Recipient applies to their Pflegekasse
Same dayOne-page Antrag auf Pflegeleistungen — the Pflegekasse is part of the Krankenkasse (TK, Barmer, AOK, etc.). Submit online or by post.
MDK assessment visit
2–6 weeksMedizinischer Dienst (MDK) or MEDICPROOF sends an assessor home. They measure six modules: mobility, cognition, behaviour, self-care, illness management, daily life. Score determines Grad 1–5.
Bescheid arrives
2–4 weeks post-visitDecision letter with Pflegegrad and start date. Retroactive to the application date, not the assessment date.
Pflegegeld lands monthly
OngoingPaid on the 1st to the recipient's bank account. Recipient forwards to family caregiver — or the Pflegekasse can pay direct to the caregiver with a one-page consent form.
Reassessment when need changes

What caregivers gain beyond the cash
Caregiver perks
- Pension credits: if you care ≥10 hrs/week across ≥2 days, Pflegekasse pays into your Rentenversicherung — up to €390/mo equivalent contributions at Grad 5
- Unemployment insurance: same threshold — caregivers stay covered even if not employed elsewhere
- Pflegezeit up to 6 months: unpaid leave from work with right-to-return, interest-free federal loan to bridge income
- Kurzzeitpflege: up to €1,854/year for respite care while you take a break
Watch out for
- Pflegegeld is tax-free only when paid to a close relative — not to a friend or distant cousin
- If you work full-time and cannot plausibly provide ≥10 hrs/week care, MDK may challenge — document shifts
- You must accept a free Beratungsbesuch every 3–6 months (Grad 2+) — skipping it freezes payments
- Cash can't be "banked" — use it or lose it, no rollover between months
